Add forms to an activity
Learn how to create forms as one of your activities on Wajooba.
You can create forms as an activity or a chapter to your course. Some facilitators use this mainly to promote their upcoming course or events and see if any of the existing members are interested to register for it.
Forms can also be used for other purposes that require you to gather your members' information. For example, asking for their guardians’ consent for an in-person activity, requiring health declaration for a fitness exercise, and so on.
We’ll help you create forms as an activity to your course.
Step 1: Choose a form to use
You can use the default registration form to gather information such as their name, email address, and phone number. You can also add or edit new registration forms to ask more information that you’ll need from your members.
- Go to Courses and select your course.
- Go to Classes & Curriculum.
- Under Curriculum, select Add activity then Form.
- Enter a title and description of the form or activity in its corresponding fields.
- From the Forms dropdown, select the registration form you want to use.
Step 2: Add contents and more information about the form
Let's add more information about the form. Follow these steps.
- Add more details or instructions about the form under the Description section. For example, "New course registration," "Health Declaration Form," or "Parent or Guardian's Consent."
- To upload a content or a YouTube or Vimeo link, select ➕ Add content under Media, then select Upload Content or Link, respectively. You can upload an image, video, PDF, or an audio file with these extensions: M4A, FLAC, MP3, MP4, WAV, AAC.
💡 You can upload multiple files as long as each file won't exceed 1 Gigabyte. If your member wants to download a file, advise them to right-click then save the file.
- (Optional) To add more resources, select ➕ Add Content under the Add Resources section of the page.
- (Optional) If you want to allow your members to repeat answering the form, select the Repeat Activity checkbox.
💡 Repeating the activity won’t override their previous answers. It will allow them to upload and answer a new form that are both available on their member profile. You can see it when you go to their profile in Contacts, then Files & Forms.
Step 3: Submit and publish activity
Make sure that all changes are being saved. Otherwise, it won't reflect on your member's end.
- To finish adding contents and save the changes, select Submit Activity.
- To make this activity available to your members, scroll up and toggle the Publish button.
💡 Select which batches can see this activity. Select the Manage Batch 👥 icon next to publish and toggle the corresponding batch.
🎉 Awesome work! The form should now be added as an activity in your course curriculum.
